The following is a listing of all the forms offered on-site for the OSAP process. 2020-2021 Academic Year 

2020-21 Supplemental Information Form for Western Students Participating in International Exchange Program (pdf)

This form is used to provide exchange study period start and end dates for your government funding application; and/or have your Official Western Letter or Third Party Form request processed to include the dates you are on exchange.

Academic Edit Letter (pdf)

The Academic Edit Letter is used during the Ontario Student Assistance Program (OSAP) process if an explanation is required regarding your Academic Progress.  

Disability Verification Form (pdf)

This form is used to obtain disability-related information from your physician or other regulated health care practitioner. This is required for your Ontario Student Assistance Program (OSAP) application if you're applying as a student with a permanent disability.

Studies at Another Institution Form  (pdf)

This form is used to indicate if you're taking courses at another institution of study that will be credited to your University of Western Ontario degree, to help in your Ontario Student Assistance Program (OSAP) assessment.

OSAP Consent to Release Information Form  (pdf)

The OSAP Consent to Release Information Form is used to allow the release of your Ontario Student Assistance Program (OSAP) information to persons other than yourself.

2019-2020 Academic Year (Includes Summer 2020)

SUMMER 2020

Registering for courses for the Summer 2020 term and applying for OSAP? Please review the following information on how to apply:

Full-time Graduate Students: All Graduate students must apply directly through the OSAP website for the summer term as there is no paper supplemental option available. Please note, if you have already received an OSAP assessment that includes the summer term (till end of Aug 2020), this option is not applicable.

UNDERGRADUATE STUDENTS: To be eligible for Full-time OSAP for the summer term, students must be enrolled in a minimum 60% course load, or in a minimum 40% course load for students who self identify as having a permanent disability. If you have a current Fall/Winter 2019-2020 OSAP Application, the summer term is an extension of this application and only requires the completion of a Summer Supplemental Form, noted below. Please note, April 1st, 2020 is the deadline to submit the Summer Supplemental Form if wishing an automatic tuition deferral. If you do not have a current Fall/Winter 2019-2020 OSAP application, and you are registered in a minimum 12 week study period for the summer term, an online 2019-2020 OSAP application is necessary in addition to the Summer Supplemental form.

Western Undergraduate Summer 2020 Supplemental OSAP Application (pdf)

Please review all details on the back of this form as it outlines the courses eligible for Full-time OSAP as opposed to Part-time OSAP. This form may not be applicable depending on your summer enrollment. If it is determined that your courses are Part-time OSAP eligible, please apply directly online through the Part-time OSAP application.

Summer 2020 OSAP Change Form (pdf)

This summer form is required when making any changes to your Summer term. For example: adding/dropping summer courses, changes to summer income, etc.
