The following is a listing of all the forms offered on-site for the OSAP process. 2023-2024 Academic Year (Includes Summer 2024)

SUMMER 2024

Registering for courses for the Summer 2024 term and applying for OSAP? Please review the following information on how to apply:

FULL-TIME GRADUATE STUDENTS: All Graduate students must apply directly through the OSAP website for the summer term as there is no supplemental option available. Please note, if you have already received an OSAP assessment that includes the summer term (till end of Aug 2024), this option is not applicable.

UNDERGRADUATE STUDENTS: To be eligible for FULL-TIME OSAP for the summer term, students must be enrolled in a minimum 60% course load, or in a minimum 40% course load for students who self-identify as having a permanent disability. If you have a current Fall/Winter 2023-2024 OSAP Application, the summer term is an extension of this application and only requires the completion of a Summer Supplemental Application. Please note, April 1st, 2024 is the deadline to submit the Summer Supplemental Application if wishing an automatic tuition deferral. If you do not have a current Fall/Winter 2023-2024 OSAP application, and you are registered in a minimum 12 week study period for the summer term, an online 2023-2024 OSAP application is necessary

2024 Full-time Summer OSAP Supplemental Application

PART-TIME OSAP Summer Term:  Please be advised that the above Summer Supplemental Application is not applicable if your summer enrollment is considered PART-TIME OSAP eligible. Less than a 60% of a full course load (or less than a 40% course load for students who self-identify as having a permanent disability) apply directly through the OSAP website for PART-TIME OSAP: https://www.ontario.ca/page/osap-ontario-student-assistance-program  The following courses/course combination, are considered PART-TIME OSAP eligible for the summer term.  Students enrolled in a mimum 40% course load and have a permanent disability have the option to apply for Full-time OSAP or Part-time OSAP.

0.5 Summer Distance 20% Course Load PART-TIME OSAP
0.5 Summer Evening 30% Course Load PART-TIME OSAP
0.5 Summer Evening & 0.5 Summer Distance 50% Course Load PART-TIME OSAP

2023-24 Supplemental Information Form for Western Students Participating in International Exchange Programs (pdf)

This form is used to provide exchange study period start and end dates for your government funding application; and/or have your Official Western Letter or Third Party Form request processed to include the dates you are on exchange.

Academic Edit Letter (pdf)

The Academic Edit Letter is used during the Ontario Student Assistance Program (OSAP) process if an explanation is required regarding your Academic Progress.  

Disability Verification Form (pdf)

This form is used to obtain disability-related information from your physician or other regulated health care practitioner. This is required for your Ontario Student Assistance Program (OSAP) application if you're applying as a student with a permanent disability.

Studies at Another Institution Form  (pdf)

This form is used to indicate if you're taking courses at another institution of study that will be credited to your University of Western Ontario degree, to help in your Ontario Student Assistance Program (OSAP) assessment.

OSAP Release of Information to Others Form  (pdf)

The OSAP Consent to Release Information Form is used to allow the release of your Ontario Student Assistance Program (OSAP) information to persons other than yourself.

2022-23 Supplemental Information Form for Western Students Participating in International Exchange Programs (pdf)
